A government commissioned report on patient safety in the NHS in England has called for a new offence of 'wilful or reckless neglect' by medical staff. We hear from its author.
Professor Don Berwick tells us why he's calling for a new offence of "wilful or reckless neglect" by NHS staff in England and a culture of "zero harm". And, we're given rare access to Ashworth High security hospital on Merseyside, which treats some of the UK's most dangerous and disturbed criminals. It's one of only four hospitals in the UK providing services for patients who require treatment and care in conditions of maximum security.
