It tells the moving stories of patients living in one of the unhealthiest areas in Britain, whose 'long-term' and 'lifestyle' conditions threaten to overwhelm the NHS. And also of the healthcare professionals trying to save them whilst at the same time fighting to fundamentally change the way their organisation works.
