Health Secretary Jeremy Hunt
Changes are being made to GPs' contracts including named doctors to coordinate care for the elderly and frail. Ministers say they're bringing back old-fashioned family doctors and it should mean fewer unnecessary admissions to hospital for up to four million patients over the age of 75.
