'Adverse events' causing birth unit deaths
A 大象传媒 Scotland investigation has found that since 2011 three women and more than 100 babies have died as a result of what are described as "adverse events" in maternity wards.
The Scottish government says the safety of mothers and babies is of paramount importance and that it is vital that lessons are learnt.
