"Tragic" deaths at Hampshire hospital
Theresa May has described the deaths of hundreds of elderly patients at a Hampshire hospital as "tragic and deeply troubling".
A major inquiry has found that the patients died because of what it says was an "institutionalised regime" of prescribing "dangerous doses" of powerful painkilling drugs when there was no medical justification.
