All this week on Breakfast, we've been taking a look at some of the problems facing Social Care and what we need to do, to make it fit for purpose in the future.

Well some of the focus is now being targetted at keeping people out of hospital in the first place and treating them at home for ongoing conditions like diabetes, remotely, using an App.

Ewa Truchanowicz is the UK Director of a company called DIGNIO which has designed an App - that allows Doctors to treat patients, from the comfort of their own home.
