Army Form B293 belonging to my Uncle, Leslie Foster from Grinshill, Shrewsbury.
On the back of the slip giving permission to be absent reads:
Instructions to a soldier, or member (other than Officer) of the A.T.S if he/she is taken ill or requires urgent medical or dental treatment while on leave.
1) Report in person to the nearest Military or other Service Medical Establishment or Emergency Medical Services Hospital.
2) If unable, because of your illness, to report in person, you must send a message to the nearest Military Medical Unit and ask for instructions.
3) If unable to do either (1) or (2) because of distance involved (over 2 miles) you may visit a Civilian Medical or Dental Practitioner, to whom you will show this pass. You will at once report the employment of a civilian practitioner to your Commanding Officer.
4) If, through sickness, you are unable to travel at the expiration of your leave and are under the care of a civilian Doctor, you must obtain a Certificate to that effect from the Doctor and forward it at once to your Commanding Officer.
To the Civilian Practitioner.
a) When a soldier is sick at the expiration of his leave and a Civilian Medical Practitioner is in attendance, the latter should be guided, in the action he takes, solely by the soldier’s fitness or unfitness to travel.
b) If fit to travel he should be instructed to return to his Unit and report sick to his Medical Officer. If unfit he should be furnished with a Certificate to that effect to be forwarded to his Commanding Officer, and at the same time be instructed to notify the nearest Military Medical establishment.
c) To obtain payment for attendance on a soldier, a Civilian Medical or Dental Practitioner should satisfy himself that the above instructions have been carried out. He should submit his claim on Army Form O.1667 which may be obtained from the local National Health Insurance Committee, and when completed should be forwarded to the A.D.M.S of the Area in which the soldier is temporarily residing for payment.
The Dental Practitioner will be paid only for strictly emergency treatment, and at the rates admissible under the National Health Insurance Act (Dental Benefit Regulations).
